Filing a Takedown Notice
To be effective under the DMCA, your notification must be in writing and include substantially all of the following:

Counter-Notification
If you believe your material was removed or disabled by mistake or misidentification, you may send a counter-notification to our designated agent including:
Identification of the material that was removed and the URL where it previously appeared;
Your name, address, telephone number, and email address;
A statement that you consent to the jurisdiction of the federal court in your district (or, if outside the United States, any judicial district in which we may be found);
A statement under penalty of perjury that you have a good faith belief the material was removed as a result of mistake or misidentification; and
Your physical or electronic signature.
Repeat Infringer Policy
In appropriate circumstances, we will terminate or restrict access for users who are determined to be repeat infringers of copyrighted works.
Misrepresentations
Please note that knowingly misrepresenting that material is infringing — or that it was removed by mistake — may subject you to liability for damages under Section 512(f) of the DMCA. If you are unsure whether the material infringes, consider consulting an attorney before filing a notice.
